Job Description Job Description Now Hiring: Continuous Quality
Improvement (CQI) Case Manager Location: Yucaipa, CA 92399 We are
currently looking for a Continuous Quality Improvement (CQI) Case
Manager to join our residential team. The Case Manager will be
responsible for: Ensuring each client receives core services and
appropriate additional treatment and services as indicated through
comprehensive assessment, and in alignment with their individual
needs and services plans. Ensuring that required documentation is
current and complete at the campus, including the intake study, the
needs and service plans, and transfer and aftercare plans.
Monitoring client files for progress towards goals, collaborate
with treatment staff and child and family teams to ensure core
services and other requirements are met. Gathering and creating
reports to include outcome measures for various traumas such as
depression, anxiety, and living skills. Skills/Requirements:
Education: A Master’s Degree in Social Work or Social Welfare,
Marriage, Family and Child Counseling, Child Psychology, Child
Development, Counseling Psychology or Social Psychology from an
accredited school is required. Demonstrated ability and leadership
through a minimum of one-year related experience in the field of
child or family welfare services or as a Social Worker required.
Must be 21 years of age or older and able to pass a physical
examination; drug clearance; criminal background check and Child
Abuse Index check. Must possess a valid California driver’s license
with clean DMV history and the ability to safely operate a motor
vehicle and be insurable by Trinity’s insurance carrier. Should
have an interest in working directly with youth in accordance with
the general requirements as established in the California
Administrative Code, Title 22. Must communicate clearly and
concisely, both orally and in writing in English. Bilingual
(Spanish/English) is a plus and strongly preferred. Pay Rate:
