Job Description:

U.S. Renal Care is seeking a compassionate and dedicated Dialysis Social Worker to join our team in enhancing the lives of patients experiencing kidney disease. This position requires an individual who can provide comprehensive social services and support to improve the emotional, psychological, and social well-being of patients undergoing dialysis therapy. The ideal candidate will be adept at assessing patient needs, developing intervention strategies, providing counseling, and coordinating with healthcare professionals to ensure holistic patient care. Conduct initial psychosocial assessments for new patients to identify emotional, social, and financial needs. Develop and implement personalized care plans in collaboration with the healthcare team. Provide ongoing counseling and emotional support to patients and their families. Advocate for patient needs within the healthcare system and community. Coordinate with external agencies to secure necessary resources and services for patients. Educate patients and families about kidney disease, treatment options, and lifestyle adjustments. Document all patient interactions and interventions accurately in their medical records. Strong interpersonal and communication skills Ability to handle sensitive information confidentially Proficient in crisis intervention Knowledge of healthcare systems and community resources Competence in case management Medical Insurance Dental Insurance Vision Insurance Life Insurance 401k Licensed Clinical Social Worker (LCSW) or eligibility to obtain licensure Certification in nephrology or willingness to obtain as required
