Practitioner.
Key Responsibilities
Evaluate patients and provide healthcare education and
counseling
Document patient care within an EHR and ensure care
coordination
Collaborate with healthcare team members to enhance patient
engagement and satisfaction
Required Qualifications
Licensed Family Nurse Practitioner (FNP)
Must be licensed in MA, FL, OH, and MD
Experience in primary care, urgent care, clinic, or hospital
settings
Remote experience preferred but not required
