Job Description:

A company is looking for a Clinical Pharmacist Market Support.
Key Responsibilities:

Conduct telephonic patient outreach and provider-facing activities to improve STARs measures
Analyze and interpret STARs and HEDIS reporting, providing insights to plan and provider groups
Perform annual medication reviews and develop action plans for patient medication adherence

Required Qualifications:

Doctor of Pharmacy degree or equivalent experience
Active, unrestricted pharmacy license in state of primary residence and Oregon
5+ years of Clinical Pharmacy Practice experience
2+ years of experience with Medicare STARs & HEDIS Measures
2+ years of experience in provider-facing interactions and data analysis
